I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, improvement of circulation, energy flow, and many other medical and non-medical reasons as may be determined by a doctor. If I experience pain or discomfort during the session, I will immediately inform my massage therapist so that pressure or strokes can be adjusted to my comfort level. I will not hold my massage therapist or Download Wellness responsible for any pain or discomfort I experience during or after the session. I understand that the services offered today may be provided in conjunction with other therapies of both a medical and a non-medical nature, however, no service provided at Download Wellness is a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness or any other medical condition unless referred or prescribed by a doctor. I affirm that I have notified my massage therapist of all known medical conditions, allergies and injuries or possible exposure to Covid 19.

 

I grant Download Wellness permission to print and use photos taken during this event for social media purposes (e.g., Facebook, Instagram, TikTok, website, etc.). By signing this form, I agree to the release of my photos for social media purposes.

 

I acknowledge that I have been informed and am fully aware that the food provided may contain allergens, including but not limited to nuts, dairy, and gluten. I understand that exposure to these allergens can result in serious allergic reactions, if I am allergic to any of the ingredients included in the food.•The food items served are as follows: turkey pinwheels, vegetable tray with ranch dip, chips, and fruit tray purchased at Costco Wholesale.
